What happened next............ 

The Trust have investigated the issues within this story and have provided feedback to family.  

Ben’s story has been shared with the Patient Experience Manager at the Health Board in order to encourage wider joint resolution.  The story has been shown at the Trust Board meeting.

As a  Trust we are continuing to work closely with trusts/health boards and hospitals throughout Wales to put robust plans in place to address issues with handover and turnaround times.
